(ii) Solicitation number Sol_75H70319Q00009 is issued as a request for quote (RFQ).

(iii) The solicitation document and incorporated provisions and clauses. Quote/Proposal packages must be submitted to the Contracting Officer via email Cordell.Bailey@ihs.gov and in the Subject Line insert, Response to Solicitation Sol_75H70319Q00009.

(iv) This acquisition is a total small business set aside.

(v) There is only one contract line item which is to provide Diabetes Education Professional Consultant services in accordance to the Performance Work Statement (PWS).

(vi) The contractor shall provide all services required in the PWS.

(vii) The period of performance is one year and will be indicated on the award document.

(viii) The attached Clauses and Provisions apply to this acquisition.

(ix) Offerors shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications­ Commercial Items, with its offer. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal.
