CDC awarded modification 00004 to task order no. 75D30118F01926 to STChealth, LLC (“STChealth”), 411 S 1st St, Phoenix, AZ 85004-2754 on September 13, 2021, in the amount of $1,984,951.12.

This notices makes available a limited sources justification that applies to this task order modification.

This task order modification was awarded under the authority of FAR 8.405-6(a)(1)(i)(A) on the basis of an urgent and compelling need.

The purpose of the original task order is to acquire immunization IT technology services to ensure the existing Louisiana Immunization Network for Kids Statewide (LINKS) immunization information system (IIS) software is maintained so that it remains operational with no interruption of workflow of current functionalities, forecasting algorithm and/or other system features.

This modification adds additional work for the contractor to build a module to produce ad hoc reports; create a COVID-19 dashboard that shows the inventory and number of COVID-19 vaccines administered; provide COVID-19 vaccination provider support services; and provide SQL script writing services.

The IIS software that is the subject of this task order operates on STChealth's proprietary software. STChealth is therefore the only vendor who can make changes to the software coding and other elements necessary to accomplish the tasks in this modification to the task order. This requirement is a time-sensitive need of the agency to provide Direct Assistance to the state of Louisiana's IIS. Fulfilling these requirements is critical to support COVID-19 vaccination efforts.

The attached limited sources justification is being made available within 30 days of the task order modification award in accordance with FAR 8.405-6(a)(2)(ii) and will remain active for 30 days in accordance with FAR 8.405-6(a)(2)(i)(B)(3).
