The purpose of this amendment is to provide question and answers from interested vendors and provide past performance evaluation questionnaire. Please see attached. The solicitation due date is extended to 30 September 2019 at 12:00 PM EST.

Q: We would request clarification with bid submission instructions / evaluation criteria. Volumes II, III and IV are clearly identified regarding content and requirements. I don't see anything in connection with Volume I. Can you clarify content and requirements of Volume I please?

Q: We have one additional question in connection with the referenced solicitation. Factor II - Past Performance (pg 44) makes reference to a Past Performance Survey. Instructions state that "Offerors shall distribute the questionnaire included in the solicitation (See D.3)..."

However, there is no questionnaire included in the Solicitation Document.
