West Lothian Health and Social Care Partnership (WLHSCP) are seeking a provider to offer a range of services which support disabled people to live independently and to fully understand the various Self-Directed Support (SDS) options available to them. This will include information, independent advice and one-to-one support to arrange and manage their own SDS package if they choose.
The overall objective of the service being to ensure the disabled person is provided advice and support to make an informed choice when exercising their SDS options and to provide assistance and support to assist the disabled person employ and manage personal assistants.
