1.  Extend the Due Date for proposal submission to FRIDAY, JANUARY 08, 2021 at 1:00 PM EST.

2.  Provide “Questions and Answers” submitted in response to the Solicitation.

3.  Attach / Upload Amendment 0001 of Solicitation Number 1605C3-21-R-00003.

Solicitation Number:  1605C3-21-R-00003

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice.  This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

The combined synopsis/solicitation information is as follows:

(1.)  Solicitation Number:  1605C3-21-R-00003

This solicitation is issued as a Request for Proposal (RFP).

(2.)  The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-02 November 23, 2020.

(3.)  The requirement shall be posted under the North American Industry Classification System (NAICS) Code of 621111— Offices of Physicians (except Mental Health Specialists).  The Small Business Size Standard for NAICS Code 621111 is $12.0 Million.

The requirement for Medical Second Opinion and Examination Services (SECOP) - Philadelphia shall be competed utilizing “Full and Open Competition”.  

PROPOSAL SUBMISSION:

Please refer to Solicitation Number 1605C3-21-R-00003 for all instructions, conditions, notices, and general guidance related to this procurement.

Offerors are expected to thoroughly review and comply with the attached solicitation. Failure to do so will be at the Offeror's own risk.

Please note the following due dates related to Solicitation Number 1605C3-21-R-00003:

1.  THE DUE DATE FOR QUESTIONS AND COMMENTS CONCERNING THIS RFP is FRIDAY, DECEMBER 18, 2020, 1:00 PM EST.

2.  ALL PROPOSAL VOLUMES MUST BE RECEIVED BY THE GOVERNMENT BY 10:00 AM EST TIME ON MONDAY, JANUARY 04, 2021.

3.  The following document is attached in support of this Request for Proposal (RFP):
