INTRODUCTION
The Mission Installation Contracting Command (MICC), Fort Knox, KY, is issuing this sources sought synopsis as a means of conducting market research to identify parties having an interest in and the resources to support the requirement for Medical Waiver Review Physician Services. The intention is to procure these services on a competitive basis.

PROGRAM BACKGROUND
The U.S. Government desires a Contractor to provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Medical Waiver Review Services for the U.S. Army Recruiting Command (USAREC) as defined in the draft Performance Work Statement (PWS).
REQUIRED CAPABILITIES
The Contractor will be responsible for reviewing military entrance physical and related medical documents, of accessions candidates disqualified by a Military and Entrance Processing Command (MEPCOM) Chief Medical Officer (CMO) in order to recommend to the command surgeon whether the Army should grant or withhold a waiver. Services include the employment of sound professional medical judgment in order to make an appropriate waiver recommendation to the Command Surgeon based on the supplied documentation. The waiver recommendation will be recorded in proper format, with any needed remarks, on the electronic SF 507-Medical Record, and the form transmitted electronically to the Command Surgeon for each case.
The three largest medical categories for waivers are ophthalmology, orthopedics, and psychiatric. Experience and demonstrated special skills in these areas by proposed contract personnel will be important in this qualifications-based selection. Workflow fluctuates throughout the year. This contract permits a means to surge capacity to perform medical waiver reviews when anticipated and unanticipated gaps in capabilities of current assets to perform medical waiver reviews occur. Recent conversion to an annual mission eliminates monthly and quarterly mission days. The Command Surgeon office is currently unable to process the increasing volume of medical waiver reviews required in the absence of a staff member, and at times of seasonal and random variations in increased demand. The Contract Waiver Surgeon should accomplish between 35 and 50 waiver review and recommendation actions in an 8-hour workday. The required services are to be performed at USAREC HQ on Fort Knox, KY.
ELIGIBILITY
The applicable NAICS code for this requirement is 621111, Offices of Physicians (except Mental Health Specialists), with a small business size standard of $12,000,000.00. The Product Service code is Q519, Medical Service/Psychiatry. Businesses of all sizes are encouraged to respond; however, each respondent must clearly identify their business size in their capabilities statement.
ADDITIONAL INFORMATION AND SUBMISSION DETAILS
A draft Performance Work Statement (PWS) and draft Technical Exhibits (TE) are attached for review.
Interested parties are requested to submit a capabilities statement of no more than five (5) pages in length in New Times Roman font of not less than 11 pitch. The deadline for response to this request is no later than 2pm, EST 29 January 2021. The sources sought number shall be referenced in the subject line of the email when responding.
No phone calls will be accepted.
This documentation must address at a minimum the following items:
Name of the firm, point of contact, phone number, email address, DUNS number, CAGE code, a statement regarding small business status (including small business type(s)/certifications(s) such as SDB, 8(a), HUBZone, SDVOSB, WOSB, etc.) and the corresponding NAICS code.
Identify whether your firm is interested in competing for this requirement as a prime contractor or not. Identify subcontracting, joint ventures or teaming arrangement that will be pursued, if any.
3. Information in sufficient detail regarding previous experience (indicate whether as a prime contractor or subcontractor) on similar requirements (include size, scope, complexity, timeframe, government or commercial), pertinent certifications, etc., that will facilitate making a capability determination. This includes at least two examples of relevant prior experience to substantiate capability to perform the tasks specified in the PWS Part 5, Specific Tasks.
4. Identify how the Army can best structure these contract requirements to facilitate competition by and among small business concerns.
